The winning combination

Treating both female and male fertility issues in an integrated multidisciplinary clinic may improve the chances of achieving a successful pregnancy

Male fertility – the topic that still isn’t talked about enough

In recent decades, there has been dramatic progress in the treatment of female fertility issues.
In vitro fertilization, egg freezing, and other procedures have become routine practices.

In contrast, male fertility issues often remain outside the spotlight.
When a couple faces fertility challenges and pregnancy does not occur despite efforts made, statistics show that in half of the cases, male factor infertility is involved—and often, treatment for both partners is required.

The right treatment begins with a precise, couple-centered approach—addressing both partners together.

Dr. Barak believes in an integrated clinic that addresses the needs of both partners.
Combining both fields allows for a complete view of the situation and a deeper understanding of the couple’s fertility challenges. It also enables the provision of second opinions for patients experiencing repeated IVF failures and can significantly contribute to improving their chances of success.

“Until the fruit of the womb ripens”

Dr. Barak and the clinic team believe that beyond professional excellence, fertility care must be surrounded by a broader perspective—one that includes humanity and compassion. As Dr. Barak puts it: “It’s essential to understand that this is an especially emotionally challenging journey.”
As the clinic’s motto, Dr. Barak chose the phrase “Until the fruit of the womb ripens”, taken from the liturgical poem “Im L’fi Becharcha Tzureinu” written by Rabbi Saadia Gaon in the 10th century. In this poem, Rabbi Saadia portrays the human experience in various circumstances, including the deep sorrow of those who are unable to bring children into the world.
“A land of salt it seems, if stricken with barrenness /
His vows pour forth like water, until the fruit of the womb ripens.”

“When I first read those words, they shook me. Rabbi Saadia Gaon refers to the inability to have children as ‘arurat’—a term rooted in the Hebrew word for barrenness—and sees it as an affliction. He compares those without children to salty soil, incapable of bearing fruit. The poem captures both despair and hope—how those who yearn for a child are willing to do anything, to vow, to weep, until their tears wash away the salt from the earth, until it yields fruit—until they are blessed with a child.”
“Day after day, I witness the tremendous efforts people make to fulfill the natural longing for parenthood. This journey is filled with challenges—physical, emotional, relational, social, and financial—difficulties of every kind. That is why, in every case that comes to me, I want to be certain that I’ve done everything within my power, using every tool that modern medicine offers—both in female and male fertility—to help them reach their cherished goal.”
“Until the fruit of the womb ripens” is not just a phrase—it is a guiding principle. It is our commitment, and we will do everything possible to make it a reality.

Questions and answers

Combining the microTESE procedure with egg retrieval allows for the use of fresh sperm and fresh eggs, which significantly improves the chances of treatment success. Sperm found in cases of non-obstructive azoospermia are often chalenging , and freezing and thawing may impair their ability to fertilize eggs and support pregnancy development. Therefore, using fresh sperm in the process increases the likelihood of achieving pregnancy.

At Dr. Shlomi Barak’s clinic, an accurate diagnosis can often be made with high certainty during the very first appointment. The diagnosis is based on a unique ultrasound method developed by Dr. Barak himself.

At our clinic, we use advanced tools based on a detailed database of all the surgeries we’ve performed to date. This data allows us to provide an accurate assessment, based not only on the general medical literature but also on our own real-world outcomes—results achieved “with our own hands.”

The decision depends on the patient’s specific situation, the quality of the sperm and eggs, and the number of embryos created. These choices require deep understanding and expertise in order to maximize success rates.

The event management process includes:

  • MicroTESE surgery: in skilled and experienced hands
  • Hormonal preparation: personalized adjustment before the procedure to increase the chances of success.
  • Egg retrieval coordination: precise synchronization with the surgery to ensure optimal results.
  • Laboratory search planning: strategic coordination for the identification of sperm cells.
  • Customized guidelines for the lab: to ensure optimal conditions for fertilization and embryo development.
  • Embryo Transfer – Optimizing the Decision Making

The search window is the time frame during which the laboratory focuses on identifying sperm cells following the surgery Precise planning of this window increases the chances of successfully identifying sperm cells.
